Prescription Drug

History
Bollinger has been providing Prescription Drug Card Plans to school boards, municipalities and commercial groups for more than 30 years. Our plan is fully insured by Monumental Life Insurance Company, which is currently rated A+ (Superior) for financial strength and operating performance by A.M. Best Co. (Ratings and Analysis 2007). All administration is done by Bollinger staff in our Short Hills, NJ location.

Plan Design
In our position as exclusive Managing General Underwriter for this program, we can offer a wide choice of benefit options to meet our customers’ needs.

Network of Pharmacies
Individuals who are covered by a Bollinger Prescription Drug Card Plan may have their prescriptions filled at any Caremark network pharmacy. Caremark is the nation’s largest pharmacy network and includes most drug store chains, as well as most independently owned stores.

Mail Order
Home delivery of prescriptions is available for those individuals who are taking maintenance medication. A 90-day supply of most drugs can be obtained by submitting a simple claim form, along with a physician’s prescription, to the Caremark mail-order facility. Refills may then be ordered by mail, by phone or over the internet.

Web Site
Everyone covered by a Bollinger Prescription Drug Card Plan may access personal Rx information a www.BollingerDrugCard.com. This secure web site links to the Caremark web site and allows members to obtain mail-order refills, as well as other useful information.

Patient Safety
Whenever a prescription is filled using Caremark, the patient’s prescription history is checked by Caremark for potentially harmful situations. These include but are not limited to, adverse drug interaction, excessive or insufficient dosage, and age appropriateness.

Discount Plan
Medications that are not normally covered by prescription drug card plans are available at a discounted price when purchased at pharmacies that participate in Caremark’s discount program. Participants include most major chain stores and many independent pharmacies.
